Milan serves as the ideal launching point for exploring northern Italy by car. While the city itself is well served by metro and trams, a rental car opens up the Italian Lakes, the Alps, and the wine regions of Piedmont and Lombardy. The key is picking up your car strategically and avoiding the congestion-charged city center.
Milan has three airports, each with rental car offices. Malpensa Airport (MXP) is the main international hub, located 50 km northwest of the city. It offers the widest selection of rental companies and competitive rates. Linate Airport (LIN) is just 7 km from the center, convenient for quick pickups, and mainly serves European flights. Bergamo Orio al Serio (BGY), used heavily by budget carriers, is 45 km east and often has the cheapest rental rates. For a Lakes trip, picking up at Malpensa puts you closest to Lake Maggiore and Lake Como.
Milan operates two restricted driving zones. Area C is a congestion charge zone covering the historic center, active Monday to Friday from 7:30 to 19:30. Entry costs €5 per day and must be paid online or at authorized retailers before midnight. Cameras at all 43 gates read license plates automatically, and rental companies will forward fines plus admin fees. Area B is a larger low emission zone covering most of the municipality that restricts diesel Euro 4 and older vehicles, as well as petrol Euro 1 and older.
The Italian Lakes circuit is the classic Milan road trip. Drive the western shore of Lake Como through Cernobbio and Tremezzo, cross to Bellagio by ferry, and return via the eastern shore. Allow two full days minimum. For wine lovers, head southwest to the Langhe in Piedmont (2 hours), home to Barolo and Barbaresco. The Bernina Express route through the Swiss Alps can also be driven (Tirano is 2 hours north), offering spectacular mountain passes.
Milanese drivers are fast and assertive, especially on the Tangenziale ring roads. Lane discipline is better than in southern Italy but still looser than in northern Europe. The A4 motorway (Turin-Venice) running through Milan is one of Italy's busiest. Tolls on Italian autostrade are paid at exit booths — keep your entry ticket safe. Fuel costs around €1.70-€1.90 per liter, and diesel is typically €0.10-€0.15 cheaper. Many rental cars in Milan are diesel or increasingly hybrid.
Winter driving near the Lakes and Alps requires caution. From November 15 to April 15, winter tires or chains are mandatory on many roads in Lombardy. Confirm your rental car has appropriate tires if traveling between these dates. Mountain passes like the Stelvio and Spluga close entirely in winter.
If staying in Milan, park at a suburban metro station's park-and-ride facility. Lampugnano and San Donato both have large, affordable lots connected to the metro. For central overnight parking, book a private garage in advance — rates spike during Milan Fashion Week (February and September), Salone del Mobile (April), and major trade fairs at Fiera Milano.

Traffic note: Milan's Area C congestion charge zone covers the entire historic center and is enforced by 43 electronic gates with cameras. Driving into Area C without paying results in a €100+ fine. Rush hour traffic on the Tangenziale (ring roads) is severe from 7:30-9:30 and 17:00-19:30. The city has an extensive tram network that makes inner-city driving unnecessary. Watch for tram tracks, which are slippery when wet and can trap narrow tires. Milan is also introducing a low emission zone (Area B) covering most of the municipality, restricting older diesel vehicles.
